It is 3-ply (B/W/B, the factory shrink wrap makes the plys difficult to see in the photo), 6 screw holes w/ center offset, brand new and sealed in its retail package. See the second listing pic for dimensions and screw hole placement. These are the same dimensions and have the same screw hole pattern as US Fender replacement tremolo guards.
